Family Questions Consent After 83-Year-Old Ontario Woman Dies During MAiD Procedure

Brigitte Stegemann, an 83-year-old grandmother from Ontario, died after receiving Medical Assistance in Dying (MAiD) at a care home in July 2026. Her family is now asking authorities to investigate whether she fully understood the procedure and gave the required consent before her death.

The case has attracted renewed attention after Belleville Police confirmed that its Criminal Investigations Division is investigating the circumstances surrounding Stegemann's death.

Stegemann died on July 10, 2026, at The Pearl care home in Cannifton, Ontario, according to accounts from her family and subsequent reports. She had been diagnosed with stage-four stomach cancer earlier in the year.

Her granddaughter, Brigitte Kranendonk, says she had been closely involved in her grandmother's care for approximately 12 years and had held power of attorney for personal care for six years.

Kranendonk has raised questions about what happened during the days leading up to her grandmother's death, particularly concerning the assessment of her capacity, the information provided to her about MAiD and whether she gave final consent.

The family maintains that Stegemann had previously rejected MAiD because of her Christian beliefs.

Family says she had previously rejected MAiD

According to Kranendonk's account, she discussed the possibility of MAiD with her grandmother after Stegemann was diagnosed with cancer.

The family says Stegemann did not want assisted dying and that her Christian faith played an important role in her decision.

Kranendonk says discussions about MAiD later resumed while she was away on a 10-day trip.

According to the family's account, Kranendonk was informed on July 3 that the care home was arranging a MAiD assessment for her grandmother. By the time she returned, an assessment had already taken place.

The family has questioned why the process moved forward despite Stegemann's earlier position.

However, these claims are allegations from the family and remain subject to investigation.

Questions surrounding her mental capacity

One of the family's main concerns is whether Stegemann had the mental capacity required to make an informed decision about MAiD.

Kranendonk says her grandmother struggled during an assessment and gave incorrect answers to some questions about her family and personal history.

She has questioned how Stegemann was ultimately determined to have the capacity to make the decision.

A report based on the family's account says Stegemann was severely hearing-impaired and sometimes extremely drowsy, circumstances the family believes should have received particular consideration during the assessment.

Kranendonk also said that during a later discussion about the procedure, she believed her grandmother appeared confused about what was going to happen.

According to the family, Stegemann became emotional after learning that the procedure would result in her death.

The family says she asked questions indicating that she was unsure about what was going to happen and expressed regret about the decision.

Granddaughter questions what happened on the day of the procedure

Stegemann's family has also raised questions about the events of July 10.

According to a detailed reconstruction based on the family's account, Stegemann spent part of the morning with relatives outside the care facility while waiting for her pastor.

The family says she was sitting in a wheelchair and eating strawberry ice cream, which they describe as one of her favourite foods.

The family says the MAiD procedure was scheduled for later that morning.

Kranendonk alleges that her grandmother did not provide a clear verbal response immediately before the medication was administered.

According to her account, Stegemann remained largely unresponsive while the medical team prepared to proceed.

Kranendonk has therefore questioned whether her grandmother understood what was happening at that moment and whether the legal requirements surrounding final consent were satisfied.

Those questions are now part of the family's request for authorities to examine the case.

Family alleges problems during the procedure

Another allegation concerns the administration of the medication.

Kranendonk told media outlets that there were difficulties establishing an intravenous line and that her grandmother suffered significant bleeding during the attempt.

She described blood getting onto Stegemann's clothing and bedding.

The family's account also alleges that the nurse attempting to establish the IV was not wearing gloves.

These details have not been independently established, and the medical professionals involved have not publicly provided a detailed response to the allegations in the reports reviewed.

The family is seeking medical records and other documentation that could establish exactly what occurred.

Family wants access to documents

The family has requested documents relating to Stegemann's MAiD process, including her eligibility assessments, signed request, information about the witness to the request, medication administration records and documentation relating to any waiver of final consent.

Kranendonk has said that obtaining those documents is important because they could help answer questions about how the decision was made and whether all required safeguards were followed.

The family has also approached authorities including Ontario's Chief Coroner's Office and the Patient Ombudsman.

Belleville Police confirm investigation

The most significant development in the case is confirmation that Belleville Police are investigating the circumstances surrounding Stegemann's death.

According to reports published in September, the investigation is being handled by the force's Criminal Investigations Division. Police have not publicly released detailed findings or announced any conclusions about whether wrongdoing occurred.

That distinction is important because the family's allegations have not been established as facts.

The investigation could potentially provide answers to questions surrounding Stegemann's capacity, consent, documentation and the circumstances of the procedure.

Until investigators complete their work, it remains unclear what conclusions authorities will reach.

What is MAiD?

Medical Assistance in Dying, commonly known as MAiD, is Canada's legal framework allowing eligible individuals to receive medical assistance to end their lives under specified conditions.

The program has safeguards intended to determine whether a person meets the legal requirements and is capable of making the relevant decision.

The Stegemann family's concerns centre on whether those safeguards were properly applied in her particular case.

The case therefore involves questions about one individual death rather than establishing that Canada's MAiD system generally operates in the same manner alleged by the family.

A family seeking answers

For Kranendonk and the rest of Stegemann's family, the central question is not simply how her grandmother died, but whether she understood what was happening and genuinely wanted the procedure to take place.

The family says Stegemann's previous opposition to MAiD, her reported difficulties during capacity assessments and what relatives describe as confusion shortly before her death deserve careful examination.

They are now asking authorities to review the medical and legal records surrounding the case.

The police investigation may provide additional information as authorities examine the circumstances.

For now, no finding of criminal wrongdoing has been publicly announced, and the allegations made by Stegemann's family remain allegations pending investigation.

The case has nevertheless generated renewed discussion about informed consent, medical decision-making and safeguards for elderly and potentially vulnerable patients.

Naija Gist Central will continue to follow the investigation and update this story when verified information is released by Belleville Police, Ontario authorities or other relevant officials.
